Chain of Custody

Definition

Chain of Custody in fulfillment is a documented record that tracks the movement, handling, and transfers of goods from receipt through picking, packing, and shipping. It captures timestamps, responsible parties, locations, and handoffs to ensure traceability, accountability, and compliance throughout the order fulfillment process.

Overview

Chain of Custody is the documented sequence of parties and locations that possess or control goods as they move through a process.

An audit-ready chain-of-custody program reduces regulatory risk, shortens dispute resolution, and improves recall response. This checklist targets fulfillment operations and helps auditors, quality teams, and operations managers confirm whether custody practices are complete, consistent, and defensible.

Pre-Audit Preparation

  • Scope Defined: Identify product categories, SKUs, and custody points within the audit scope (receiving, storage, staging, shipping, returns).
  • Retention Policy: Confirm document retention periods align with industry regulations and contracts.
  • System Access: Ensure auditors have read access to WMS/TMS logs, scanned signatures, and sensor data.

Documentation Checks

  • Custody Event Records: Verify there are signed or electronic records for each defined custody transfer (including time, personnel, and location).
  • Integrated Evidence: Check that related evidence (photos, temperature logs, carrier PODs) is attached to each custody record.
  • Change Log: Confirm changes to custody records (edits, reversals) are audited with user ID and rationale.

Process And Controls Verification

  • Defined Transfer Points: Observe and confirm staff perform and record custody transfers at designated points.
  • Access Controls: Review role-based permissions in WMS and physical access controls for high-value/regulated zones.
  • Dual Authorization: Test transactions requiring two-person verification for compliance or high-risk moves.

Technology And Data Integrity

  • Audit Trails: Verify scan histories, signature captures, and sensor recordings are complete and timestamped.
  • System Clocks: Confirm system clocks across WMS, TMS, and mobile devices are synchronized to prevent timestamp disputes.
  • Backups: Ensure custody records and sensor logs are backed up according to policy and recoverable for the retention period.

People And Training

  • Training Records: Check that staff handling custody events have documented training and competency records.
  • Accountability: Confirm disciplinary and escalation procedures are known and applied when custody protocols are breached.
  • Shadow Audits: Review results of internal spot checks or mock recalls that test custody response.

Sample Audit Activities

Walk a sample of shipments from receipt to shipping: inspect physical markings, scan histories, signed transfer forms, and any sensor data. Select returns and quarantined items to confirm custody was maintained during inspection and disposition. Compare carrier proof-of-delivery to outbound custody records to ensure handoffs were captured.

Common Findings And Remedies

  • Missing Signatures: Remedy by introducing mobile signature capture and mandatory fields in WMS.
  • Unlinked Records: Remedy by standardizing identifiers and enforcing scan-before-move policies.
  • Unsynchronized Timestamps: Remedy by implementing network time protocol (NTP) sync across systems and devices.

In short, the Chain of Custody audit checklist ensures fulfillment operations can produce consistent, auditable evidence of who controlled goods and when—reducing liability, speeding recalls, and keeping regulated shipments compliant.
