Overview

Delivery Failure Rate is the percentage of deliveries that fail or require reattempt, exception handling, or return. It captures whether the delivery process completed successfully on first attempt, not only whether it arrived within a scheduled window.


Operations teams commonly track both Delivery Failure Rate and On-Time Delivery (OTD). They are complementary but measure different problems: OTD measures timeliness, while Delivery Failure Rate measures successful completion. Understanding their differences prevents mistaken conclusions — for example, a carrier may show excellent OTD while also forcing many returns that don’t count as late but still harm customers and raise costs.


What Each Metric Measures


Define each metric before comparing them in dashboards. Use consistent event codings from your WMS/TMS and carrier scans.


  • Delivery Failure Rate: Percentage of shipments requiring reattempts, exceptions, or returns because the first delivery attempt did not complete a successful handoff.
  • On-Time Delivery: Percentage of shipments delivered within the promised delivery window or service-level time (e.g., next-day, two-day). Late deliveries are counted as misses regardless of whether they are successful on first attempt.


How They Diverge In Practice


Scenarios that separate the metrics:


  • Late But Successful: A parcel delivered 48 hours late but accepted by the customer counts as an OTD miss but not a failure.
  • On-Time But Failed: A driver arrives within the promised window but the recipient is not there or access is denied; this is an OTD success in some systems (if timestamped) but a delivery failure because the handoff didn’t occur.
  • Multiple Attempts: Redeliveries can create repeated on-time or late events; the business impact is often increased cost even when final delivery meets a promised window.


Why You Need Both Metrics


Using both KPIs provides a fuller view of customer experience and cost:


  • Customer experience: Customers care about both timeliness and not having to chase a package or schedule redeliveries.
  • Cost control: Redelivery and exception handling have fixed handling and transport costs that OTD alone won’t show.
  • Carrier performance: A carrier scoring high on OTD might still generate expensive failures; combining metrics reveals trade-offs.


Interpreting Combined Outcomes


Combine the metrics in a small matrix for daily scorecards:


  • High OTD / Low Failure Rate: Ideal — deliveries are timely and complete. Focus on incremental cost reductions and scaling capacity.
  • High OTD / High Failure Rate: Timely attempt but poor completion. Investigate access issues, customer communications, and address data.
  • Low OTD / Low Failure Rate: Deliveries eventually succeed but are late. Optimize routing, cut dwell times, and review carrier scheduling.
  • Low OTD / High Failure Rate: Systemic execution problems — prioritize root-cause analysis and immediate corrective action.


Example: Reading A Carrier Scorecard


Carrier A shows 97% OTD and 6% Delivery Failure Rate. Carrier B shows 93% OTD and 1.2% failure. Which is better? For urgent B2B customers Carrier A’s speed might matter, but for high-volume residential runs Carrier B will likely reduce customer contacts and re-delivery costs. The correct choice depends on order profile, SKU characteristics, and cost-to-serve calculations.


Practical Reporting Recommendations


To avoid misinterpretation:

  • Label definitions: Document which event codes feed each metric (successful handoff, late scan, exception types).
  • Segment reports: Report by customer type (B2B vs B2C), delivery mode, and zone to surface meaningful patterns.
  • Include cost per failure: Convert failures to dollars to show financial impact alongside OTD percentage.


In short, the Delivery Failure Rate measures the share of deliveries that require reattempts, exception handling, or return. Pair it with On-Time Delivery to understand both timeliness and completion success; use combined analysis to select carriers, set SLAs, and prioritize operational fixes that improve customer experience while controlling redelivery costs.
