The intake form is a structured questionnaire used during onboarding to capture the information a third-party logistics provider needs to price, design, and operate warehousing and fulfillment for a merchant.
The intake form converts conversations into executable data. It is the first practical step after a merchant and a 3PL agree to explore a partnership. Good intake forms reduce back-and-forth, speed integration with a warehouse management system (WMS), identify special handling or compliance needs, and set expectations for lead times, service windows, and costs.
What The Form Typically Collects
The fields on an intake form vary by service model (fulfillment, distribution, cross-dock) but most capture these categories:
- Product Data: SKU numbers, dimensions, weights, packaging types, unit-per-case, and hazardous material flags.
- Volume Forecasts: Average and peak weekly/monthly volume, seasonality windows, and launch schedules.
- Sales Channels: Marketplaces, direct-to-consumer, retail, wholesale — and channel-specific requirements like EDI or carrier choices.
- Service Requirements: Pick/pack tolerances, kitting, returns handling, custom packaging, labeling, and dropship rules.
- Locations & Routing: Origin points, final destinations, delivery SLA expectations, and special carrier contracts.
- Operational Rules: Cutoff times, order batching logic, fulfillment windows, and customer-specific KPIs.
- Compliance & Safety: Certificates, country-of-origin, temperature control needs, and hazardous goods documentation.
Why It Matters To Operations And Pricing
Operations require precise inputs to size labor, racking, slotting, and equipment. Pricing models rely on volume profiles, cubic utilization, and process complexity. If SKU dimensions or cartonization are wrong, storage and shipping estimates become inaccurate and margins erode. Intake forms bridge sales conversations and operational reality: they trigger WMS configuration, carrier integrations, and SLA definitions.
How The Form Impacts Systems Integration
Modern 3PLs link intake data to systems: a WMS for inventory and pick logic, a TMS for carrier selection, and an ERP via EDI or API for order flow. Early capture of EDI segments, API keys, and channel mappings reduces integration cycles. The intake form should explicitly request identifiers (GTIN, SKU), preferred order transmission method (SFTP, API, EDI), and sample files to validate formats before go-live.

- Basic Intake: Short questionnaire for simple e-commerce brands with limited SKUs and low complexity.
- Advanced Intake: Detailed technical appendix for high-volume merchants, regulated products, or custom packaging flows.

Practical Example
A mid-size apparel merchant completing an intake form might provide: 1,200 SKUs with dimensional data, monthly order volume of 8,000 units with a November–December peak to 25,000, a requirement for polybagging and hang-tagging, and a need for returns processing tied to the merchant’s customer-service workflow. The 3PL uses that data to recommend slotting density, temporary labor plans for peak, estimate costs per pick, and specify label templates for channel compliance.
Tips For Merchants Completing The Form
- Prepare Data Files: Assemble SKU lists, dimensions, and sample labels as CSV/Excel before starting the form.
- Be Honest About Peaks: Understating seasonality creates undercapacity and missed SLAs during peak periods.
- Document Exceptions: Note any fragile, regulated, or temperature-sensitive SKUs separately rather than burying them in descriptions.
- Share Process Maps: If you have an order flow diagram or returns flow, attach it to avoid misalignment.
